Carnitine palmitoyltransferase 1 (CPT1), also known as carnitine acyltransferase I (CAT1), is an outer mitochondrial membrane enzyme essential for breaking down long-chain fatty acids for energy. The integral membrane enzyme mediates the formation of acyl carnitines by catalyzing the transfer of the acyl group of a long-chain fatty acyl-CoA from coenzyme A (CoA) to L-carnitine, which is essential for the transport of long-chain fatty acids across the inner mitochondrial membrane and constitutes the rate-limiting step in beta-oxidation of long-chain fatty acids. Three isoforms of CPT1 exist (CPT1A, CPT1B, and CPT1C) that differ in tissue distribution and sensitivity to inhibition by malonyl-CoA. While CPT1A predominates in lipogenic tissues such as liver, CPT1B is more abundant in tissues possessing a high fatty acid oxidative (FAO) capacity, namely, heart and skeletal muscle. In addition, CPT1A is more sensitive to allosteric inhibition by malonyl-CoA, allowing liver FAO to be tightly regulated by dietary intake. During the fed state, CPT1A is inhibited by malonyl-CoA, which is a signal of fatty acid synthesis in the fed state. In the fast state, fatty acids imported from the adipose tissue generate energy through beta-oxidation. Genetic variants in the CPT1A gene lead to a deficiency in the enzyme, resulting in hypoglycemia, hypoketosis, and a buildup of fatty acids in multiple tissues. The CPT1 activity assay is based on CoA-dependent reduction of INT to formazan, which exhibits an absorption maximum at 492 nm and allows for measurement of CPT1 activity in cell/tissue lysates.
